| Medical Coverage |
| Medical Plans are self-funded through the Boys & Girls Club Workers Association Insurance Trust.
This plan uses many of the largest nationwide PPO (Preferred Provider Organization) Networks.
There are 5 unique plans to help fit your needs.
- Club Select - $500.00 individual deductible or $1,000.00 family.
- Club Choice - $750.00 individual deductible or $1,500.00 family.
- Club Advantage - $1,250.00 individual deductible or $2,500.00 family.
- Club Value - $1,500.00 individual deductible or $3,000.00 family & $100.00 for prescription.
- Club Saver - $2,000.00 individual deductible or $4,000.00 family & $100.00 for prescription.
Each plan carries prescription benefits through CVS Caremark, the nation’s largest pharmacy benefit manager.
These Medical Plans are not Grandfathered Plans under the new Health Care Reform Legislation. The plans will be fully compliant with all the requirements of the Patient Protection and Affordable Care Act (PPACA) as of January 1, 2011. This means these plans will:
- Pay 100% of preventive services performed by a Network provider.
- Have no lifetime limit
- Have an annual limit of $2 million until 2014.
- Cover dependent children through age 25.
- Not apply any pre-existing conditions limits on children under age 19.
- Be compliant with all appeals and reporting requirements.

Dental with Vision Plans are self-funded through the Boys & Girls Club Workers Association Insurance Trust.
Choose The Dental Or Vision Professional Of Your Choice
No Deductibles
- Preventive dental procedures 100% reimbursed; ALL other dental procedures (such as fillings, crowns, partials) reimbursed 70%
- Orthodontia expenses reimbursed 70% (regardless of age)
- Eye exams reimbursed 70%
- Eyeglass frames & lenses reimbursed 70%
- Contact lenses reimbursed 70%
- $1,250 annual limit per person - $2500 annual limit per family

Dental Plan is self-funded through the Boys & Girls Club Workers Association Insurance Trust.
Freedom to choose any dental professional working within the scope of his/her license.
- Preventive dental procedures reimbursed at 100%
- Diagnostic and Basic dental procedures reimbursed at 70% (deductible applies)
- Major dental procedures reimbursed at 60% (deductible applies)
- Orthodontia reimbursed at 50% (available for dependents through age 18)

| Short Term & Long Term Disability |

Short Term Disability Plans are self-funded through the Boys & Girls Club Workers Association Insurance Trust. Long Term Disability is underwritten by UniCare Life and Health Insurance Company.
Low cost disability coverage provides sick leave benefits
- Both Short & Long-Term combined offer the most comprehensive, uninterrupted salary continuation protection for Club employees
- Both Plans pay 66-2/3% of gross income
- Rehabilitative benefit included
- Short Term Disability covers disabilities lasting up to 6 months - including maternity
- Short Term Disability payments begin on the 8th day of disability
- Long Term Disability covers disabilities lasting longer than 6 months - until age 65 if necessary

Life Insurance is underwritten by UniCare Life and Health Insurance Company
Affordable Life Insurance Coverage - including Accidental Death & Dismemberment Benefits - for Employees
Coverage for spouses and children, too (life only, no AD&D)
- Life insurance that is equal to 1, 2, or 3 times an employee's salary; or in $1,000 increments, beginning at $10,000
- Employee coverage guaranteed up to $100,000
- Clubs may offer both options, if desired
- Pays double for accidental death; pays additional for loss of sight or extremities
- Employees can purchase additional coverage above what Club provides
